Knowing all you can about your health and your family’s medical history is an important part of staying healthy. Because LifeBridge wants you to take an active role in your health, we are offering several easy ways for you to know as much as you can about preventing life-threatening diseases.

An especially important aspect of prevention is understanding if you or your family are predisposed to a particular health concern based on your personal medical information, your health habits and family history. By taking a few minutes to register for your private user name and password with give you access to a series of free online health risk assessments to help you know more how to better manage your personal health.

It only takes a few minutes and can save your life. When the assessment is completed, print your results and share them with your physician. You will be informed if you are at an elevated risk for a specific disease. If you do not have a physician and would like to find one, please call our Physician Referral Line at 410-601-WELL.

Once you are registered, you can come back as often as you like to take additional risk assessments. It is one more way LifeBridge Health helps you be “in the know” about your health.
